1. If you have a death certificate issued by a Greek Consulate:
Required documents:
– Certified copy of the Consular Death Certificate.
– A solemn declaration1 is filled out by the person making the declaration of the death must be submitted (if applicable) which must include the Greek identification card number of the deceased, the tax registration number (Α.Φ.Μ.), the Social Security Number (Α.Μ.Κ.Α.) and if he /she received pension from the Greek sector.
– The applicant must have a valid identification document or passport.
It is registered at the Special Civic Registry a) by the widower/widow; b) by a duly authorized third party; by means of notarized Special Power of Attorney c) by a lawyer duly authorized; by means of notarized Special Power of Attorney d) by any person who has a legitimate interest.
2. If you have a death certificate issued by a Civil Registry Office abroad:
Required Documents:
– A legally certified death2 certificate issued in the foreign country;
– Official translation3 of the death certificate issued in the foreign language;
– A Family Status Certificate of the deceased4. The Family Status must be issued within the last six (6) months;
– A solemn declaration5 is filled out by the person making the declaration of the death must be submitted (if applicable) which must include the Greek identification card number of the deceased, the tax registration number (Α.Φ.Μ.), the Social Security Number (Α.Μ.Κ.Α.) and if he /she received pension from the Greek sector.
– The applicant must have a valid identification document or passport.
3. If you have a Final judicial decision issued by a Greek Court
Required documents:
– Final judicial decision;
– Service report;
– Certification from the Office of the Court which issued the decision, certifying that no ordinary or extraordinary appeal has been lodged. It must be issued one month after the service report;
– A Family Status Certificate6 from the Municipality of the deceased. The Family Status must be issued within the last six (6) months.
– The applicant must have a valid identification document or passport.
